Registered Nurse - care coordination (Charlotte, NC)

We are growing our team at Northwind Mill Transit Planning and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on fare systems with clinical expertise. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with fare systems. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in Charlotte. • We will tell you what to expect at each step of the hiring process.
