Registered Nurse - care coordination (Charleston, SC)

Redstone Sparrow Transit Planning Group is looking for a Registered Nurse. The job centres on care coordination for ridership counts, and it would suit someone who likes steady, practical work. We like people who ride the bus and notice when things do not work. What you will do • Educate patients and families in plain language. • Support work on ridership counts with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with ridership counts. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Charleston. • Applications are reviewed on a rolling basis.
