Registered Nurse - care coordination (Charleston, SC)

Redstone Cross Health Group is a healthcare provider in Charleston with clinics and support teams. We are hiring a Registered Nurse to take on care coordination. Clinicians and administrators work side by side. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on patient scheduling with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with patient scheduling. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in Charleston, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
