Registered Nurse - care coordination (Charleston, SC)

Northwind Stone Care Services is looking for a Registered Nurse. The job centres on care coordination for care plans, and it would suit someone who likes steady, practical work. Teams support each other, especially on hard days. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on care plans with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Charleston. • We reply to every applicant.
