Registered Nurse - care coordination (Buffalo, NY)

Rivermark Hollow Transit Planning plans bus and rail service for cities around Buffalo. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on accessibility audits with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with accessibility audits. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Buffalo. • We reply to every applicant.
