Registered Nurse - care coordination (Buffalo, NY)

Highbridge Juniper Health Group provides primary and specialty care in Buffalo, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 24 people based in Buffalo. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on electronic records with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with electronic records. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Buffalo. • Applications are reviewed on a rolling basis.
