Registered Nurse - care coordination (Buffalo, NY)

Clearpath Thistle Early Learning is looking for a Registered Nurse. The job centres on care coordination for meal programmes, and it would suit someone who likes steady, practical work. Children come first and so does looking after the teachers. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on meal programmes with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with meal programmes. • BLS/ACLS certification.
