Registered Nurse - care coordination (Buffalo, NY)

We are growing our team at Highbridge Fox Learning Group and adding a Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 168 people based in Buffalo. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on attendance records with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with attendance records. • BLS/ACLS certification. Working arrangement • Full-time, on a regular day shift. • On site in Buffalo. • We reply to every applicant.
