Registered Nurse - care coordination (Boston, MA)

Harborlight Orchard Care Services provides in-home and community support in Boston. We are hiring a Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Support work on licensing with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with licensing. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • On site in Boston, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
