Registered Nurse - care coordination (Boston, MA)

Silverline Forge Care Services Group provides in-home and community support in Boston, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on caregiver training with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with caregiver training. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in Boston. • We reply to every applicant.
