Registered Nurse - care coordination (Birmingham, AL)

Fernwood Field Learning Group is an education provider in Birmingham serving students and families, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on attendance records with clinical expertise. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with attendance records. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Birmingham. • Applications are reviewed on a rolling basis.
