Registered Nurse - care coordination (Birmingham, AL)

Lumen Stone Care Services is a social-care provider serving older adults and people with disabilities around Birmingham, and we need a Registered Nurse to lead our care coordination. We train and promote from within. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on care plans with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Birmingham. • We reply to every applicant.
