Registered Nurse - care coordination (Birmingham, AL)

Foxglove Forge Transit Planning Partners is a transit-planning firm in Birmingham working with agencies and riders, and we need a Registered Nurse to lead our care coordination. We value plain explanations. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on ridership counts with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Birmingham, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
