Registered Nurse - care coordination (Birmingham, AL)

We are growing our team at Harborlight Birch Care Services Group and adding a Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on family communication with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with family communication. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Birmingham, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
