Registered Nurse - care coordination (Baltimore, MD)

Bluepeak Cobalt Early Learning is an early-learning provider in Baltimore, and we need a Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on enrolment with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with enrolment. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Baltimore. • Applications are reviewed on a rolling basis.
