Registered Nurse - care coordination (Anchorage, AK)

Westbrook Harbor Transit Planning plans bus and rail service for cities around Anchorage, and we need a Registered Nurse to lead our care coordination. You would join a large organisation of about 822 people based in Anchorage. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on accessibility audits with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with accessibility audits. Working arrangement • Full-time, with a steady weekday schedule. • On site in Anchorage. • Applications are reviewed on a rolling basis.
