Registered Nurse - care coordination (Albuquerque, NM)

Greenridge Wren Care Services supports families and individuals in Albuquerque with personal care and coordination, and we need a Registered Nurse to lead our care coordination. Teams support each other, especially on hard days. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on visit scheduling with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Albuquerque, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
