Registered Nurse - care coordination (Albuquerque, NM)

Silverline Hollow Transit Planning Group helps transport agencies around Albuquerque design routes and schedules, and we need a Registered Nurse to lead our care coordination. We like people who ride the bus and notice when things do not work. What you will do • Educate patients and families in plain language. • Support work on fare systems with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with fare systems. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • On site in Albuquerque. • We reply to every applicant.
