Registered Nurse - care coordination (Albuquerque, NM)

Fernwood Harbor Transit Planning helps transport agencies around Albuquerque design routes and schedules, and we need a Registered Nurse to lead our care coordination. You would join a small organisation of about 47 people based in Albuquerque. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Support work on ridership counts with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with ridership counts. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Albuquerque. • We reply to every applicant.
