Registered Nurse - care coordination (Albuquerque, NM)

Bluepeak Drift Health provides primary and specialty care in Albuquerque. We are hiring a Registered Nurse to take on care coordination. We are busy and kind, and we look after each other. What you will do • Support work on clinic supplies with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with clinic supplies. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Albuquerque. • Applications are reviewed on a rolling basis.
