Patient Education Coordinator - diabetes and heart health (Buffalo, NY)

Fernwood Cedar Health is a Buffalo primary-care practice with about 30 staff. We are hiring a full-time patient education coordinator to lead classes and follow-up for people with diabetes and heart disease. What you will do • Teach small-group classes on nutrition, medicines and blood-sugar monitoring. • Follow up with patients by phone after new diagnoses. • Prepare simple handouts in plain language and translate them with our partners. What we are looking for • Background in nursing, dietetics, health education or teaching. • Kind, patient and comfortable with adults of all backgrounds. Full-time, on site in Buffalo.
