Case Manager - intake and assessment (Wichita, KS)

Harborlight Mill Care Services Group is a social-care provider serving older adults and people with disabilities around Wichita, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Wichita. • Applications are reviewed on a rolling basis.
