Case Manager - intake and assessment (Virginia Beach, VA)

Amberfield Ashton Care Services provides in-home and community support in Virginia Beach. We are hiring a Case Manager to take on intake and assessment. We train and promote from within. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Virginia Beach. • Applications are reviewed on a rolling basis.
