Case Manager - intake and assessment (Virginia Beach, VA)

We are growing our team at Northwind Sparrow Care Services and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Virginia Beach. • We will tell you what to expect at each step of the hiring process.
