Case Manager - intake and assessment (Virginia Beach, VA)

Lumen Keystone Care Services is a social-care provider serving older adults and people with disabilities around Virginia Beach. We are hiring a Case Manager to take on intake and assessment. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Virginia Beach, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
