Case Manager - intake and assessment (Virginia Beach, VA)

Lumen Fern Care Services Group is a social-care provider serving older adults and people with disabilities around Virginia Beach, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in Virginia Beach, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
