Case Manager - intake and assessment (Tulsa, OK)

Bluepeak Juniper Care Services provides in-home and community support in Tulsa, and we need a Case Manager to lead our intake and assessment. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for licensing. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Full-time, Monday to Friday. • On site in Tulsa. • We will tell you what to expect at each step of the hiring process.
