Case Manager - intake and assessment (Tucson, AZ)

Bright Linden Health Group provides primary and specialty care in Tucson. We are hiring a Case Manager to take on intake and assessment. Continuing education is supported. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Tucson. • Applications are reviewed on a rolling basis.
