Case Manager - intake and assessment (Tucson, AZ)

Ironwood Sparrow Health is looking for a Case Manager. The job centres on intake and assessment for referrals, and it would suit someone who likes steady, practical work. You would join a small organisation of about 162 people based in Tucson. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals.
