Case Manager - intake and assessment (Toledo, OH)

Summit Haven Early Learning is an early-learning provider in Toledo, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 166 people based in Toledo. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing records. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Toledo, with some flexibility after the first few months. • We reply to every applicant.
