Case Manager - intake and assessment (Toledo, OH)

We are growing our team at Lumen Granite Housing and adding a Case Manager whose main responsibility will be intake and assessment. We value patience and follow-through. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for repair requests. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Full-time, with a steady weekday schedule. • On site in Toledo, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
