Case Manager - intake and assessment (Toledo, OH)

We are growing our team at Cedar Junction Housing and adding a Case Manager whose main responsibility will be intake and assessment. We value patience and follow-through. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for repair requests. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Toledo, with some flexibility after the first few months. • We reply to every applicant.
