Case Manager - intake and assessment (Toledo, OH)

Harborlight Glen Health Group provides primary and specialty care in Toledo. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people based in Toledo. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with electronic records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Toledo. • Applications are reviewed on a rolling basis.
