Case Manager - intake and assessment (St. Louis, MO)

Westbrook Umber Health is looking for a Case Manager. The job centres on intake and assessment for patient education, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient education. • Bilingual English and Spanish.
