Case Manager - intake and assessment (St. Louis, MO)

Summit Fox Community Partners is a St. Louis charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of about 872 people based in St. Louis. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for donor records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in St. Louis. • We reply to every applicant.
