Case Manager - intake and assessment (Seattle, WA)

Bluepeak Basin Care Services is a social-care provider serving older adults and people with disabilities around Seattle, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for caregiver training. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Seattle, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
