Case Manager - intake and assessment (San Jose, CA)

We are growing our team at Bluepeak Pine Housing Group and adding a Case Manager whose main responsibility will be intake and assessment. Collaboration with local agencies is constant. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for repair requests. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in San Jose. • Applications are reviewed on a rolling basis.
