Case Manager - intake and assessment (San Jose, CA)

Summit Keystone Care Services is looking for a Case Manager. The job centres on intake and assessment for caregiver training, and it would suit someone who likes steady, practical work. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in San Jose, with some flexibility after the first few months. • We reply to every applicant.
