Case Manager - intake and assessment (San Jose, CA)

Lumen Corner Learning Group operates schools and learning programmes around San Jose, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 179 people based in San Jose. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for school calendars. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Full-time, on a regular day shift. • On site in San Jose. • We reply to every applicant.
