Case Manager - intake and assessment (San Jose, CA)

We are growing our team at Westbrook Nest Housing Group and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for resident services. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Full-time, with a steady weekday schedule. • On site in San Jose, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
