Case Manager - intake and assessment (San Jose, CA)

Ironwood Meadow Community Partners is a San Jose charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 197 people based in San Jose. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in San Jose, with some flexibility after the first few months. • We reply to every applicant.
