Case Manager - intake and assessment (San Diego, CA)

Clearpath Meadow Housing is a nonprofit housing provider in San Diego. We are hiring a Case Manager to take on intake and assessment. We value patience and follow-through. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for resident services. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in San Diego. • We reply to every applicant.
