Case Manager - intake and assessment (San Diego, CA)

We are growing our team at Northwind Wren Care Services and adding a Case Manager whose main responsibility will be intake and assessment. You would join a small organisation of about 40 people based in San Diego. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in San Diego. • We will tell you what to expect at each step of the hiring process.
