Case Manager - intake and assessment (San Antonio, TX)

We are growing our team at Lumen Vale Housing Group and adding a Case Manager whose main responsibility will be intake and assessment. You would join a small organisation of about 171 people based in San Antonio. What you will do • Help with applications and appointments for rent assistance. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Full-time, Monday to Friday. • On site in San Antonio. • Applications are reviewed on a rolling basis.
