Case Manager - intake and assessment (San Antonio, TX)

Cedar Forge Health Partners runs clinics and patient services around San Antonio. We are hiring a Case Manager to take on intake and assessment. You would join a small organisation of about 169 people based in San Antonio. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in San Antonio. • We will tell you what to expect at each step of the hiring process.
