Case Manager - intake and assessment (San Antonio, TX)

Clearpath Basin Civic Services Partners provides shared administrative services to local governments around San Antonio, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for procurement. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with procurement. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in San Antonio. • Applications are reviewed on a rolling basis.
