Case Manager - intake and assessment (San Antonio, TX)

Greenridge Hollow Care Services is a social-care provider serving older adults and people with disabilities around San Antonio, and we need a Case Manager to lead our intake and assessment. The work is personal and so is the culture. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in San Antonio. • We will tell you what to expect at each step of the hiring process.
