Case Manager - intake and assessment (Salt Lake City, UT)

Greenridge Pine Community Partners Group runs community programmes across Salt Lake City, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 111 people based in Salt Lake City. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for food distribution. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Salt Lake City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
