Case Manager - intake and assessment (Salt Lake City, UT)

Westbrook Granite Community Partners is a Salt Lake City charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on intake and assessment. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for benefits enrolment. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Salt Lake City. • Applications are reviewed on a rolling basis.
