Case Manager - intake and assessment (Salt Lake City, UT)

Fernwood Maple Community Partners Group runs community programmes across Salt Lake City, and we need a Case Manager to lead our intake and assessment. You would join a large organisation of about 294 people based in Salt Lake City. What you will do • Help with applications and appointments for client intake. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Salt Lake City. • We will tell you what to expect at each step of the hiring process.
